- through our websites, social media pages, and other digital platforms we operate or control, whether directly or indirectly via third parties, that reference this Privacy Notice, including www.kindtcmclinic.com.sg (the “Online Channel”).
- when you become a Kind TCM Clinic member through the purchase of any packages online and/or in-stores.
- when you visit any of our roadshows and external events; and
- through other data collection points that reference this Privacy Notice (collectively, the “Channels”).
Depending on the Channels you use and the nature of your interactions with us, you may provide us with certain types of personal information including:
- Contact Information such as name, email address, contact number, postal address.
- Demographic Information such as age, date of birth, gender.
- Medical Information such as surgical operations, existing known medical conditions.
- Government-Issued ID Information such as NRIC, driver’s license, passport.
- Content such as photos, comments, feedback.
- Signature at point of registration in clinics.
- Employment Application Information such as employment history, education, proof of eligibility to work in Singapore and any other information if you apply for a job with us.

Providing your personal information to us is voluntary on your part. If you choose not to provide us certain information, we may not be able to offer you certain services or products.
