At Kind TCM Clinic Pte Ltd (the “Company”), we respect your privacy and are committed to protecting your personal information. and strive to provide all customers (“you”) with professional and effective TCM services. This Privacy Notice describes the types of personal information we obtain, how we may use and retain that personal information, with whom we may share it, and how you may exercise your rights and choices with respect to our processing of that information in accordance with the Personal Data Protection Act (PDPA) of Singapore.    • when you become a Kind TCM Clinic member through the purchase of any packages online and/or in-stores.
   • when you visit any of our roadshows and external events; and
   • through other data collection points that reference this Privacy Notice (collectively, the “Channels”).

   Depending on the Channels you use and the nature of your interactions with us, you may provide us with certain types of personal information including:

   • Contact Information such as name, email address, contact number, postal address.
   • Demographic Information such as age, date of birth, gender.
   • Medical Information such as surgical operations, existing known medical conditions.
   • Government-Issued ID Information such as NRIC, driver’s license, passport.
   • Content such as photos, comments, feedback.
   • Signature at point of registration in clinics.
   • Employment Application Information such as employment history, education, proof of eligibility to work in Singapore and any other information if you apply for a job with us.
   • Any other personal information you provide through the Channels.

   Providing your personal information to us is voluntary on your part. If you choose not to provide us certain information, we may not be able to offer you certain services or products.

   We collect personal information about you in different ways and from various sources. This includes, without limitation, when you visit our websites, contact us via e-mail or phone, visit us at any of our clinics, attend our roadshows and events and so on. We use your personal information to provide you with medical services, including diagnosis, treatment, and follow-up care. This may involve sharing your information with our healthcare professionals, including physicians, doctors, nurses, and allied health professionals, who are involved in your care.

   We may also use your personal information for the following purposes:

   • To communicate with you about our services, including appointment reminders, information about your transactions, test results, and health education materials.
   • Responding to you and providing support including to address your enquiries and feedback and any other communications.

   • Managing career opportunities including process and assess your job application, conducting recruitment and other talent management purposes.

   • Prevention and protection including protecting all customers, employees, identify and prevent any unlawful activities, protect, and defend the company against legal claims and other liabilities.
